Athletic Clearance
Hillcrest High School athletic clearance is completed through Home Campus. As part of the clearance process, athletes must have a current sports physical completed within the last 12 months.
* All required clearance information must be submitted and approved before participation in practices, tryouts, or competitions.

Complete & Upload the Sports Physical
Download and print the required sports physical form. Make sure all required sections are completed and signed before uploading the form to Home Campus.
Download Physical FormPhysicals must be completed by a Medical Doctor (MD), Doctor of Osteopathy (DO), or Physician Assistant (PA-C). Chiropractic physicals are not accepted per AUSD policy.
Complete and Submit the Application
Review the Home Campus application to make sure all required information, signatures, and documents have been completed. Submit the application once all requirements are finished.
Wait for Clearance Approval
Submitting the application does not automatically clear a student athlete for participation. The completed application and physical must be reviewed and approved before the student athlete is cleared to participate.

A Sports Physical Alone Does Not Complete Athletic Clearance
The sports physical is one required part of the athletic clearance process. The entire Home Campus application must be completed, submitted, and approved before a student athlete is cleared to participate.
