Sports Physicals

To try out for and participate on the school’s athletic teams, student-athletes must have a sports physical dated within the past 12 months.

Physicals may be submitted to the school’s athletic director; please provide a copy rather than the original.

Where to Get a Physical: You can obtain a sports physical from your family physician, a local clinic, CVS, or Walgreens.

  • Physicals must be completed by a Medical Doctor (MD), Doctor of Osteopathy (DO), or Physician Assistant (PA-C).
  • Chiropractic physicals are not accepted per AUSD policy.
  • Physicals are valid for one year from the date of the exam.
  • If you choose to use a physical form other than the district-provided version, it must clearly state "Cleared to Play Sports."
  • All physicals must include the official stamp of the physician's office.
